Question:

Which of the following options should the nursing assistant report to the nurse?

A When providing HS care, the nursing assistant measures and disposes of 100cc of cloudy yellow urine from a patient’s bedside toilet.
explanation

Because the urine is cloudy, it is important for the nursing assistant to report on these changes. Cloudy urine may be a sign of other health issues. Urine should be between clear and a light yellow. Because urine that is clear yellow is safe, each of the three other answers is all usually normal. Additionally, requesting for assistance to make it to the bathroom is perfectly fine.

There are definitely a few mistakes in this app, some of the answers are just plain wrong so make sure this is not your only source of study material. Other than that this app is very helpful.

I would love to give this app 5 stars but in the Nursing Skills section it does not allow you to go on to part 7. After you get to part 6 it either just never loads or it tells you to go to part 7 and does absolutely nothing.
