The nursing assistant needs to help the patient to the bathroom first. Following this, the nursing assistant have to then inform the nurse on staff about the rise in the patient’s temperature. Option A is wrong, because the patient needs assistance walking to the bathroom. They are at greater risk of slipping and injuring himself or herself if they attempt to do this on their own.  Option B is incorrect as the nursing assistant needs to inform the nurse on staff about the change in temperature. Option D is incorrect as providing medication is beyond the nursing assistant’s scope of practice.

There are definitely a few mistakes in this app, some of the answers are just plain wrong so make sure this is not your only source of study material. Other than that this app is very helpful.

I would love to give this app 5 stars but in the Nursing Skills section it does not allow you to go on to part 7. After you get to part 6 it either just never loads or it tells you to go to part 7 and does absolutely nothing.
